Co-op / Class Conflict Form

Fill out this form if you will not be able to attend meetings for a semester due to Co-op or a Class Conflict.

Explanation of attendance impact:
If you are on co-op you will be marked as such for the semester and will be removed from the meeting quorum requirement. You will remain in good standing while on co-op.
If you have a class conflict, you will be marked as excused for the semester. This will remain in affect for the semester, but your absence will still be counted against meeting quorum. You will remain in good standing because of the class conflict.

Note: Society representatives should not fill out this form. A society is not able to go on co-op and should send an alternate representative to council meetings.

Semester which the co-op or class conflict takes place

Full name, first & last as it appears on council member records

Select your current position on council. Society representatives should send an alternate representative if they are unable to attend meetings for a semester.

Is there another email you would prefer to be contacted at while on Co-op? If class conflict, leave blank.

If on Co-op, Where? If class conflict, leave blank.

List the Department, Number, and Section of the conflicting class. (I.E. EAC 101-06). This class must run between 12:20-1:00pm on Thursdays.
